Introduction
New care delivery models including ACOs and PCMH continue to proliferate across the country spurred by private payers interest in coordinated care management and the introduction of the Medicare Shared Savings Program introduced by the Patient Protection and the Affordable Care Act. Over the past 12 months CMS has announced 32 Pioneer ACOs and more recently the first Medicare ACOs. This activity has been mirrored by commercial health insurers revealing extensive plans for ACO development, groups such as Aetna, United Health group and Blue Cross Blue Shield continuing to pursue performance based contracts with providers across the country. With the uncertainty surrounding the Supreme Court decision now behind us the continued expansion of the Commercial and Medicare ACO market continues to accelerate.

Accountable Care IT Readiness
